Accessing your medical records

You can access your medical records in a number of ways:

1. On-line - via the internet

To arrange this - please complete this form (application form for on-line access to records). Once completed - bring it to reception - with your photo ID (e.g. photo driving licence).

For patients aged 13 years or under - parents can be provided with access to appointment booking / repeat prescriptions / immunisations/ laboratory test results. This access will be removed when the young person reaches the age of 13 years.

2. By asking for a copy of them

To arrange this - please complete this form (application for a copy of medical records).

Once completed - bring it to reception and we will arrange to have your medical records copied.

Please note that it can take up to 28 days to provide copies of medical records.

3. By making an appointment to arrange to see them

You can contact the surgery to make an appointment to come into the surgery to view your records.
